    Description
    We are currently seeking a Full-Time Physical Therapist to join our Avante Team

    Avante at Leesburg, a Skilled Nursing Facility, is seeking a Physical Therapist to join our team of long-term care professionals.

    This is a vital role in a facility with a positive community reputation, a history of successful outcomes, and an experienced cohesive team.

    This is a skilled nursing facility, with an In-house therapy department.

    Daily Pay

    Provides and directs Physical Therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication.

    Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with department procedures.

    • Conducts screenings of residents at regular intervals to determine the need for intervention/treatment.
    • Evaluates residents to obtain data necessary for treatment planning and implementation.
    • Bachelor of Science in Physical Therapy from an accredited program.
