- Molina Healthcare Services (HCS) collaborates with members, providers, and a diverse team to evaluate, facilitate, plan, and coordinate an integrated care delivery for individuals with high necessity levels.
- HCS employees ensure that patients are moving towards desired outcomes through quality and cost-effective care, tailored to their illness severity and service location.
- Provides phone, administrative, and data entry assistance for the Case Management team.
- Conducts initial evaluations of case levels to support Case Management allocations.
- Analyzes data to pinpoint member needs and collaborates with the Case Manager to execute care strategies.
- Schedules member meetings with the team as necessary.
- Evaluates members following Molina's guidelines, aiding clinical Case Management staff in determining suitable medical services.
- Organizes essential services as per the member's benefit plan.
- Enhances case management services by fostering internal and external communication.
- Handles member and provider communications.
- Required Education: HS Diploma or GED.
- Required Experience: 1-3 years in an administrative role within healthcare.
- Preferred Education: Associate degree.
- Preferred Experience: 3+ years in an administrative healthcare role, Medical Assistant background is a plus.
