    As a Per Diem Home Health Physical Therapist, you'll have the opportunity to make a profound impact on patient lives.

    Job Responsibilities:

    • Engage deeply with patients by assessing conditions and meticulously documenting progress with warmth and professionalism.
    • Provide personalized, top-tier care from vital signs to daily activities.
    • Utilize your expertise to improve therapeutic, rehab, and functional statuses, adding joy and progress to patient lives.
    • Suggest devices or tools to enhance patient lifestyles.
    • Address patient concerns and fulfill requests with grace and professionalism.
    • Work seamlessly with our team, including RNs, Occupational Therapists, and Physicians, to tailor care plans.
    • Empower patients and families with effective home health care strategies.

    Requirements:

    • Hold a State Physical Therapy license from the Physical Therapy Board of California.
    • Communicate effectively and manage time efficiently.
    • Maintain a valid driver's license for patient visits.
    • Keep your CPR certification and TB clearance current.

    Essential Experience:

    • At least 1 year of experience in Physical Therapy within home health settings.

    Salary & Benefits:

    $65.00 - $81.00 per hour
