    Responsibilities Facilitates communication between the patient, family members and healthcare providers to ensure patient satisfaction and quality of care. Reviews primary diagnosis, co-morbidities, and clinical documentation in HCHB to validate or advise care center on applicable care needs.

    Assists clinical staff with the identification of clinical findings which may indicate the need for additional services or an alternative care setting to maximize the patient's health condition.

    Ensures appropriate referrals from home health, hospice, palliative and or personal care are coordinated effectively and efficiently. Runs all applicable patient tracking, activity reports and documents in the system. Assists with internal or external transfer of patients between care settings. Works collaboratively with peers to support coordination efforts and patient care needs. Complies with policies, procedures, and regulatory mandates. Performs other duties as assigned. Facilitates communication between the patient, family members and healthcare providers to ensure patient satisfaction and quality of care.
