- Deliver hands-on patient care for assigned individuals.
- Execute all technical aspects of dialysis procedures for designated patients as per instructions. Administer and oversee treatments following the current plan or supervisor's guidance.
- Track and record dialysis treatment parameters on flow sheets. Document other relevant patient care details in individual records.
- Promptly report any significant updates, changes in patient status, or equipment issues to the supervisor.
- Contribute to developing and implementing a teaching plan to educate patients and families on end-stage renal disease, dialysis therapy, dietary requirements, and medications.
- Support emergency operational protocols.
- Administer intravenous medication in line with physician orders and nursing practice regulations.
- Conduct and document pre, interim, and post-treatment assessments of patient well-being.
- Take essential CPR measures during cardiac or pulmonary arrest; respond to dialysis treatment-related emergencies.
- Operate all dialysis-related equipment safely following correct procedures.
- Provide minor troubleshooting when needed.
- Maintain a clean, safe, and hygienic environment in the treatment area of the dialysis facility.
- Address all blood spills promptly according to HDS/DCC Bloodborne Pathogen Control Policies.
- Effectively handle all emergency equipment as per standard operating procedures.
- Engage in scheduled staff meetings.
- Contribute to short-term and long-term patient care plan discussions.
- Enhance knowledge and skills in current dialysis practices by participating in scheduled in-service sessions.
- Serve as a point of reference for colleagues, aiding in coordinating care plans with the Physician, Dietitian, and Social Worker.
- Perform other duties as assigned.
- Promote and embody the values and customer service benchmarks of HDS/DCC.
- Deliver exceptional customer service to all staff, clients, and vendors.
- Establish and nurture effective relationships through timely and efficient communication.
- Take proactive steps to address, resolve, and follow up on customer service matters promptly.
- Graduate of a recognized School of Practical Nursing (LPN, LVN).
- Hold a current CPR Certification.
- Proficient in PC operations, thriving in a high-paced environment; experience in Microsoft Office Suite: Outlook, Word, and Excel is vital.
- Possess strong presentation and communication skills.
- Demonstrate productivity and resourcefulness, meeting deadlines effectively.
- Preferably, a minimum of 6 months of hemodialysis experience.

MISSION AND FOCUS:Embraces the purpose and values of Home Dialysis Services & Dialysis Care Center, LLC (HDS/DCC).
Commits to upholding the HDS/DCC Policies and Procedures, ensuring compliance with state and federal regulations.
The role of the Licensed Practical Nurse revolves around delivering exceptional care to Hemodialysis patients at the designated In Center(s).
KEY RESPONSIBILITIES:
Patient Care:
The physical demands and work environment outlined are typical for this role. Reasonable adjustments may be accommodated for individuals with disabilities.
Day-to-day tasks include desk work, computer use, and interactions with clients. Travel between facilities and locations is part of the role, with occasional staff meeting travel.
The work settings resemble those of a healthcare facility, with controlled air temperature and moderate noise levels. Exposure to infectious materials is possible in dialysis centers or patients' homes.
A capability to lift 30 pounds when necessary is required.
