    Job Description

    We seek an experienced Registered Nurse (RN) for our team. Responsibilities include conducting client visits, assessing conditions and environments, developing Activities of Daily Living (ADLs) plans based on client needs, and evaluating caregiver performance.

    Key Responsibilities:

    • Conduct comprehensive client visits to assess health conditions and living environments.
    • Develop individualized care plans including ADLs based on client needs.
    • Monitor and document client progress, making necessary adjustments to care plans.
    • Provide education and support to clients and families regarding health management and ADLs.
    • Evaluate caregiver performance to ensure quality care according to established care plans.
    • Collaborate with healthcare professionals and caregivers for holistic client care.
    • Maintain accurate and timely medical records and documentation.
    • Ensure compliance with healthcare regulations and standards.
    • Participate in ongoing training and professional development for best practices in home care nursing.

    Qualifications:

    • Valid and current Registered Nurse (RN) license in South Carolina.
    • Minimum 1-2 years of nursing experience, preferably in home health care or related field.
    • Strong clinical skills and experience in conducting health assessments.
    • Excellent communication and interpersonal skills.
    • Ability to develop and implement ADL plans tailored to individual client needs.
    • Experience in evaluating and providing feedback to caregivers.
    • Ability to work independently and manage time effectively.
    • Compassionate and patient-centered approach to care.
    • Valid driver's license and reliable transportation.
