    Occupational Therapist Job Description

    We are seeking a dedicated Part-time or consistent PRN Occupational Therapist to join our team at Vertis Therapy's Assisted Living Facilities in the Ft Wayne, IN area.

    Key Responsibilities:

    • Evaluate and treat patients according to physician orders using therapeutic procedures to improve independence, safety, and quality of life.
    • Develop and implement individualized treatment plans based on patient needs and containing achievable functional goals.
    • Conduct thorough functional assessments of patients, including home/vocational and equipment needs.
    • Revise treatment plans as needed throughout the treatment process.

    Requirements:

    • Master of Science degree in Occupational Therapy or Bachelor of Science degree in Occupational Therapy and five years of direct practice experience.
    • Current license/certification in the state of practice.
    • Prefer current registration with the American Occupational Therapy Certification Board.
