Job Title: Family Nurse Practitioner – Primary Care

We are seeking compassionate and experienced Family Nurse Practitioners to join our Primary Care teams in Dundee, Penn Yan, and Seneca Falls, New York.

This is a full-time opportunity for individuals committed to providing high-quality, patient-centered care through long-term relationships.

The ideal candidates will be passionate about improving the health and wellbeing of the community, providing comprehensive care to individuals and families, and being part of a supportive organization.

Key Responsibilities:

Provide primary care services in an outpatient setting, including routine check-ups, health screenings, and preventive care.
Diagnose and treat common medical conditions and manage chronic diseases.
Perform physical exams, prescribe medications, and interpret diagnostic tests.
Develop treatment plans and educate patients on healthy lifestyle choices, disease prevention, and management.
Collaborate with healthcare professionals to ensure the best possible outcomes for patients.
Maintain accurate and up-to-date patient records in the EMR system.

Work Schedule:

Full-time position, Monday through Friday.
Standard office hours with potential for flexible scheduling.

Qualifications:

Board Certification as a Nurse Practitioner (FNP).
New York State (NYS) Nurse Practitioner license.
Graduate of an accredited Nurse Practitioner or Physician Assistant program.
Current BLS and ACLS certifications.
Prior experience in a primary care setting preferred.
