Job summary
The Community Health Worker supports patients and their families with educational resources about their diseases and helps to navigate the diagnosis and treatment process. Provides age-appropriate information during evaluation. Identifies support groups for patients and coordinates scheduling removes obstacles by arranging appointments, referrals, transportation etc.Benefits
