    Job Title: PRN Case Manager Registered Nurse
    Job Type: PRN/Hourly | M-Sun 8a-4:30p
    Department/Unit Overview
    The Case Management team plays a vital role in enhancing the care of hospitalized patients. Our dedicated team consisting of RN Case Managers, Social Workers, Utilization Review Staff, and Discharge Planners ensures that payer requirements are met while crafting individualized care plans for each patient. We coordinate closely with the interdisciplinary team and community resources to maximize the chances of patient success.

    Qualifications and Requirements
    Applicants must possess a current state RN license and hold either an associate or bachelor's degree from an accredited nursing institution. Additional qualifications include:
    • BLS Certification
    • A minimum of three years of experience in acute care as a Professional Nurse, or significant acute care hospital Case Management experience
    • Previous case management experience is preferred
    • Case management certification (CP) is preferred
