    JOB SUMMARY

    The Psychiatric Nurse Practitioner works with patients to ensure the highest quality of services regarding mental wellness. Incumbent is a key member of the mental health team and works together with the Director of Behavioral Health to provide services for mental health and substance abuse. The Psychiatric Nurse Practitioner will help bridge the gap to provide efficient services for persons suffering from mental health and substance abuse disorders.

    PRIMARY FUNCTIONS
    1. Assist the Licensed Mental Health Counselor, Licensed Clinical Social Worker, and/ or Psychiatrist to provide psychosocial needs of patients and families.
    2. Perform patient assessments to evaluate their state of mental health, completing nursing process, collaborating with counselor and providing safe, accurate and clinically competent care to the patients at True Health Centers.
    3. Perform a variety of diverse and complex telephonic and face-to-face care management responsibilities and ensures that all service needs are met through monitoring of treatment plans, crisis plans, and inter-agency collaboration.
    4. Build strong rapport with patients and help make accurate assessments of their problems whether mental/emotional or physical in the case of substance abuse.
    5. Documents clinical assessments and coordination of care in the medical management information system in a timely manner that meets regulatory and accreditation standards.
    6. Promotes patient mental wellness of children, adolescents, and adults.
    7. Strive to promote patient self-advocacy and self-determination.
    8. Advocate for the patient to facilitate positive outcomes for the patient, health care team, and payers.
    9. Adheres at all times to general and clinical guidelines.
    10. In all activities works to ensure linkage and integration of behavioral health and medical care.
    11. Participate in staff outreach and off-site health care programs requiring Behavioral Health services as requested by the Chief Medical Officer
    12. Represent True Health in the community.
    13. Participate in at least one internal clinical committee
    14. Assess patients with substance abuse disorders and participate in Medication Assisted Treatment therapy (MAT)
    15. Assess and diagnose patients with ADHD disorders and participate in medical management and treatment.
    16. Work independently solving problems within the purview of their authority.
    17. Adhere to all HIPAA and confidentiality laws as appropriate.
    18. Other duties as assigned.
    19. Examine patients and record history
    20. Adhere to proper charting protocols
    21. Discuss with Physicians and other health care professionals to prepare comprehensive plan
    22. Order or execute tests or images as needed
    23. Submit health care plan and goals of individual patients for periodic review by Physician
    24. Analyze results of tests, diagnose and report to patient
    25. Administer or prescribe treatment and medications
    26. Promote health by advising patient on diet, hygiene and prevention of disease
    27. Inoculate and vaccinate as required
    28. Refer to specialist as required for patient's well-being
    29. Willing to provide support at behavioral health offices
    30. Report deaths and contagious diseases to governmental authorities
    31. Other responsibilities as assigned
    EDUCATION AND EXPERIENCE
    1. Must have and maintain current State of Florida license in good standing
    2. Current National Board Certification
    3. Master's degree in Nursing, with a specialty in Psychiatric Nursing
    4. Minimum of three years' experience in a clinical setting (preferred)
    KNOWLEDGE, SKILLS, AND ABILITIES
    1. Mental Health experience preferred
    2. Excellent communication, negotiation and advocacy skills.
    3. Current Florida RN Licensed, active and in good standing.
    4. Current DEA, active and in good standing.
    5. Computer competency is required
    6. Effective written, verbal and electronic communication skills
    7. Show initiative, creativity and problem-solving ability
    8. Ability to collaborate with colleagues to provide quality healthcare
    ADDITIONAL QUALIFICATIONS
    1. Bilingual is a plus (English / Spanish/Creole)
    2. Degree in a behavioral health field such as psychology or social work is a plus.
    RELATIONSHIP REPORTING
    1. Reports to the Chief Medical Officer and Psychiatrist
    PHYSICAL REQUIREMENTS
    1. Ability to stand and or walk for extended periods of time
    2. Ability to view a computer screen for extended periods of time
    3. Ability to perform repetitive hand and wrist motions for extended periods of time
    4. Ability to communicate effectively with patients and staff
