- Assess the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment.
- Diagnose and treat various mental, emotional and behavioral disorders.
- Develop a plan of care either individually or with input from the patient, patient family/significant others, physicians and other members of the healthcare team.
- Modify care plans to reflect changes in the patient's status.
- Refer patients to other services to improve their well-being. These referrals may include community services, treatment programs, support groups, psychiatrists, or other mental health providers.
- Properly documents pertinent information in regard to the patient's plan of care.
- Work with the multi-disciplinary team to develop a discharge plan in order to provide further care, services, and referrals in order to ensure continuity of care.
- Comply with federal, state, local, and clinical program/facility standards.
- Act within the scope of the Mental Health Practice Act.
- Master's Degree from an accredited institution (degree will be verified)
- Licensure in the State of Utah as a Clinical Mental Health Counselor.
- Successful completion of examination through either the National Counseling Examination (NCE) or the National Clinical Mental Health Counseling Examination (NCMHCE).
- Behavioral Health experience.
- Computer skills (email, word processing, and spreadsheets).
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
- Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on
