    RN Care Manager

    Applicants need to be local due to on site meetings.

    Will support offices in Cohoes and Waterford.


    This position is an ambulatory care registered nurse who will work within a hybrid model focusing on our trinity health medical group patients who are admitted to one of the St.

    peters Health Partners Hospitals (Samaritan/SPH).

    The position will require an in person presence on M,W,F with the ability if desired to work remotely T, TH.

    This position will focus on disease education, identifying and mitigating social influencers of health, coordinating needed services and collaborating with the provider offices to ensure integrated care through the healthcare continuum and improved health outcomes of our patients.

    AD will be grandfathered and must meet performance standards.

    Previous experience required in care management at least one of the following settings:
    disease management, home health care, hospital or physician office setting. 5 years of RN experience in Care Management required. Prior electronic medical record (EMR) experience required. Previous project leadership experience required. Possess strong community resource knowledge. Ability to work independently, setting priorities to coordinate care plan efficiently.

    Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
