    Iowa mask only.
    Positive TB requires a chest xray within the last year for Michigan, Indiana, Connecticut and Massachusetts. All other regions within 5 years.
    Resume, Skills Checklist within a year, 2 Professional References required.
    Not required for non-clinical. For clinical contractors, the agency will provide this checklist.
    10-panel drug screen completed within 30 days of start


    Panel required:
    methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates.
    All engaged contractors must have page 27 signed form on file in Trinity VMS.
    Standard i9 completed by the agency.
    All engaged contractors must have page 5 signed form on file in TrinityVMS.
    Signed within days prior to start.
    Attach copy of specialty exam must be completed within a year.
    Proof of vaccine Sept 2025 booster, declination or exemption (with agency letterhead). Declination not accepted for CA or IL. End date of October 2026.

    Attached scanned proof of 2 vaccinations with dates or primary source IgG titer with lab print-out to candidate's record in Trinity VMS.

    Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later.

    Hep B titers will NOT be accepted. Proof of vaccination must be uploaded into Trinity VMS.
    Proof of vaccine, declination or exemption (with agency letterhead). Declination not accepted for CA or IL.
