    Home Health Physical Therapist PT PRN

    Position Overview:
    Aveanna Healthcare is expanding rapidly due to recent acquisitions and currently seeks a Physical Therapist (PT) to join the home health care team. In this role, you play a vital role in helping patients recover from illness or injury in the comfort of their own homes. Weekend availability for 2-3 days a week is needed for Collier county.

    Essential Job Functions:
    - Perform evaluations to assess mobility, coordination, strength, and current function level.
    - Administer tests to determine goals and outcomes.
    - Develop a physical therapy plan and document progress.
    - Deliver services as per physician orders.
    - Maintain care plan, adjust goals as needed, and plan for discharge.
    - Educate patients and caregivers on post-discharge exercise plans.
    - Coordinate services with healthcare team members.
    - Document accurately and ensure compliance with standards.
    - Communicate effectively with staff and families.
    - Supervise support personnel where applicable.
    - Complete OASIS assessments as required.
    - Adhere to policies, regulations, and licensure standards.

    Requirements:
    - Physical Therapy degree from an accredited program.
    - Current state license as a Physical Therapist.
    - Valid CPR certification.

    Preferred:
    - Strong organization and communication skills.
    - Attention to detail.
    - Time management abilities.
    - Proven relationship-building skills.
    - Experience in Home Health Therapy and OASIS documentation.

    Note: Medicare and Medicaid acceptance require compliance with health-related regulations, including vaccination and testing mandates, with exemptions permitted under specific circumstances.
