    GENERAL SUMMARY AND PURPOSE:

    As an Outpatient Cardiovascular Pulmonary Rehabilitation Social Worker, you will provide advocacy, education, assessments, and interventions to patients and families. The goal is to ensure optimal care and active patient participation. Collaboration with medical staff and external organizations is key to developing specific treatment plans and program participation strategies that lead to high-quality clinical outcomes.

    SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

    • Must hold a license as a Clinical Social Worker (LCSW) in Idaho.
    • Must obtain LCSW licensure in Oregon within one year of employment.
    • At least one year of post-licensure experience in behavioral health or integrated care is required.
    • Strong familiarity with behavioral medicine and evidence-based treatments for medical and mental health conditions is preferred.

    ESSENTIAL FUNCTIONS:

    • Incorporate and demonstrate the organization's Mission, Vision, and Values in all actions and decisions.
    • Provide appropriate interventions for patients, families, and caregivers, such as Social Screenings and mental health support.
    • Engage in care coordination activities for patients requiring social work or complex case management assistance.
    • Collaborate with the healthcare team to optimize resource allocation.
    • Accurately record assessments, plans, and interventions in the EMR system.
    • Coordinate care with mental health and substance abuse facilities.
    • Ensure program retention and patient participation by overseeing timely completion.
    • Adhere to established patient choice policies.
    • Coordinate with physicians and legal entities on matters like guardianships and psychiatric holds.
    • Participate in multidisciplinary case conferences to address the holistic needs of patients and their families.
    • Conduct initial psycho/social exams of patients to ensure program compliance.
    • Attend department meetings and other relevant teams or committees as necessary.
    • Adhere to the National Association of Social Work (NASW) Code of Ethics and State Licensure Board regulations.
