- Assists with ""Activities of daily living"" means the functions or tasks for self-care which are performed either independently or with supervision or Activities of daily living include mobility, transferring, walking, grooming, bathing, dressing, undressing, eating, and toileting, per the Plan of Care.
- Assists with "Independent Activities of daily living" which may include light housekeeping, shopping, laundry and meal preparation, per the Plan of
- Accompany clients to appointments (transportation is client's responsibility unless driving waiver is signed and per the Plan of Care)
- Provides assistance with self-administered, pre drawn and medication reminders, per Plan of
- Understands and maintains infection control and safety measures
- Reports any and all changes in the client's condition to the Care Manager or
- Completes the Activity Records and accurately documents the care provided
- Performs all activities as directed by the Care Manager or BOM
- Has a positive relationship with clients and families
- Has positive client satisfactions
- Meets all deadlines for health screening requirements and
- Encourage maximum independence to facilitate rehabilitation as determined EP and
