- Obtains accurate, current patient medication information through direct patient, family or caregiver interviews, examination of medication bottles, review of patient provided lists, review of computer generated external medication histories information, review of information from other healthcare facilities, and/or by contacting pharmacies to obtain and/or clarify medication information.
- Record medication information into the home medication section of the hospital electronic medical record (EMR) and updated existing information as needed to ensure accuracy of the patient home medication list. This information includes, but is not limited to, correct medication name, medication formulation, dose, route directions, schedule, date of last dose taken and source used to obtain information.
- Record the patient's preferred outpatient pharmacy in the electronic medical record.
- Screen for potential drug interactions and adverse effects.
- Verify patient allergies with reactions.
- Communicate with pharmacists regarding any issues, concerns or discrepancies that are found.
- Must have good working knowledge of medications, including brand and generic names, most common doses prescribed, most common frequency/directions prescribed, and most common scheduled or PRN indication for which the medication is prescribed.
