Description

General Position Summary:

The Referral Coordinator manages community specialist and diagnostic testing referrals as

requested by provider staff. Referral appointments are scheduled by coordinating care with

the patient and are managed according to the Referral Policy. The Referral Coordinator

works with the patient population, JHC staff, and the outside provider community,

supporting the provider team in a patient-centered model of care.

Essential Duties and Responsibilities:

Clinical Duties:

1. Supports medical staff in providing patient-oriented care while accessing consultants

and specialty appointments as ordered.

2. Ensures that providers have all needed information for the referral, including but not

limited to the correct ICD-10 codes, questionnaires, and the proper amount of

documentation.

3. Trains new providers on the referral process and how to place a referral and DI when

needed.

4. Maintains signed provider forms for all referral processes. Alerts providers with new

forms as needed.

Specific Duties and Responsibilities:

1. Employee will abide by the Code of Conduct as documented in the Employee

Handbook.

2. Must demonstrate a personal and professional commitment to Johnson Health Center

and its mission.

3. Adjusts to changes as set by management to meet organizational needs.

4. Manages all medical referrals and DIs to required specialist practices as ordered by

providers by asking patient preference, using current knowledge of insurance

restrictions, and working with specialty office staff.

5. Obtains insurance pre-certifications for referrals and DI as required by the patient's

insurance policy.

6. Manages and maintains referrals according to Referral Policy guidelines and within set

timeframes.

7. Works alongside JHC Pharmacy staff to make sure that all documentation needed to

capture 340B is obtained.

8. Creates referrals and DI as needed in order to capture Diabetic Eye Exam, Mammogram

and Colonoscopy results to meet HRSA requirements.

9. Provides appropriate customer service and answers patient inquiries.

10. Provides information and support to medical providers regarding new or existing

patient referrals made by the Johnson Health Center provider.

11. Trains all new providers on the referral and DI processes.

12. Keeps referral information current by documenting patient contacts and specialist

office notifications.

13. Works alongside the Associate Director of Health Information to ensure that the

providers "How To" guide is current and up to date.

14. Helps with coverage in the Records Department as needed and requested by the

Associate Director of Health Information.

15. Performs other duties as delegated by the ADHI.

Staff Relations/Team Building:

1. Works with supervisors, JHC staff, and community provider offices to identify and

resolve referral process workflow issues to maintain efficient, patient-centered care.

2. Updates providers on any changes with community provider offices regarding whether

they are not taking new referrals or are now accepting new services.

Community Involvement:

1. Maintains contact with specialty offices and manages workflow processes that support

patient-centered care.

2. Meets with new community partners regularly to ensure a seamless process for both

parties.

Qualifications:

1. High School graduate or equivalent with experience in medical office operations.

2. Referral experience preferred. Strong customer/patient service focus.

3. Well-developed verbal & written communication skills.

4. Demonstrated ability to work well on a team.

5. Strong follow-up and follow-through with delegated tasks and responsibilities.

Physical Demand and Working Environment:

Fast-paced office setting with travel to other offices often. Lifting and/or exerting force up

to 5 pounds occasionally, with frequently moving of objects. Work requires speaking,

sitting, bending, walking, standing, hearing, and stooping, kneeling, and repetitive motion

with certain activities. 8 hours of constant computer usage. OSHA low-risk position.
