- Collaborate and facilitate patient discharge plans for post inpatient care
- Reassess and adjust care plans based on patient needs
- Conduct patient assessments, considering illness, mental state, and family dynamics
- Document data accurately and ensure compliance with regulations
- Connect patients and families with community resources
- Advocate for patients with physicians, insurance companies, and community agencies
- Provide clinical supervision to junior social workers
- Adhere to University policies and procedures
- Education: Master's Degree in Social Work
- Experience: Minimum 1 year
- Certification: BLS Certified (within 3 months of hire)
- Conduct systematic psychosocial assessments
- Participate in on-call rotation for after-hour calls
- Advocate for psychosocial-spiritual concerns
- Thoroughly document in medical records
- Implement social work interventions
- Collaborate with healthcare team for patient care plans
- Provide education, counseling, and support to patients and families
- Facilitate discharge planning
- Assist with financial issues
- Address survivorship issues
