    Description

    Up to $2,000 Sign-On Bonus $$ # Summary The Nurse Aide Rehab Specialist completes training in the areas of care for those living with dementia (including crisis prevention, dementia live and Hand in Hand) as well as annual education as required for dementia care and participation in Woodside Workshops as requested.

    In addition, The Nurse Aide Rehab Specialist is trained in care of post-surgery orthopedic patients and those who have been recently hospitalized for medical issues such a CHF and COPD including use of Stop and Watch, transfer techniques, restorative nursing and completion of therapy #homework#.

    Person-centered care will be provided so as to maximize highest level of functioning and return to previous home setting in a safe and calm environment Essential Duties and Responsibilities Identifies and suggests innovative approaches and helps implement performance improvement opportunities.

    Complies with Presbyterian SeniorCare Network policy and procedure in accordance with governing laws, regulations, regulatory agencies, accreditation bodies and facility standards.

    Consistently demonstrates individual and team behavior that supports our person-centered culture, the Woodside philosophy, and the mission, values and behavior standards of the Network.

    Maintains confidentiality. Other duties may be assigned. 1.

    Following each resident/patient plan of care provides direct care to improve resident/patient functional status in person-centered manner using approaches specific to persons living with dementia.

    2.

    Performs nursing rehabilitation techniques as taught including implementing restorative nursing tasks (therapy homework) to maximize functional improvement as quickly as possible.

    3.

    Active participant and leader in activities and restorative nursing tasks to engage residents/patients under the direction of the therapy team and recreation therapist to maximize attainment of goals.

    4. Assists with any admissions, transfers, or discharges of residents. 5. Promotes safe, clutter-free, homelike environment that fosters free movement. 6. Remains calm, cooperative and supportive while on duty especially in emergency and unsettling situations. 7. Provides care and support to residents/patients at all times regardless of medical or behavioral issues. 8. Applies progressive system of verbal/non-verbal approaches to de-escalate tense situations. 9.

    Participates in or leads huddle process to proactively bring up and solve problems faced in daily work particularly around safety.

    10. Assists residents with meals, monitor consumption. 11. Takes vital signs, reports observed changes to the nurse. 12. Follows and promotes infection control policies and procedures.

    Reports any exposure to blood or body fluids infectious material and hazardous chemicals in accordance with established policies and procedures.

    13. Records accurate and legible information about resident/patient care in appropriate sections of resident/patient record. 14.

    Communicates effectively with neighborhood coordinator, nurse, and therapy team throughout the shift and collaborates with care team including performing a thorough shift-to-shift report upon leaving.

    Supervisory Responsibilities None Education and/or Experience Certificate in good standing from Nurse Aide Registry. Minimum age 18.

    Willingness to complete all required training including crisis prevention, Dementia Live, and Hand in Hand, restorative nursing, and all CNA courses on 1 Learning Place.

    Preference given to those with previous experience in caring for those living with dementia as well as experience in short-term rehab Computer Skills Certifications, Licenses, Registrations Certificate in good standing from Nurse Aide Registry.

    Summary
