    Main Function

    Under the direction of the Administrator the Total Joint Navigator is responsible and accountable for planning, organizing and monitoring operations of the Total Joint Replacement Program to achieve and improve patient through-put, outcomes, provisions of care and services. As the clinical experience liaison and consultant for orthopedic providers, caregivers and families this position is responsible for planning, coordination, integration, education and the ongoing evaluation of delivery of care to the specialty orthopedic population in need of total joint replacement across the ASC service area.

    The Navigator is responsible for assuring delivery of integrated, orthopedic care from the initial contact with the patient, pre-operative, peri-operative and post-operative settings. The total Joint Navigator drives care coordination by actively providing consultative screening, evaluation and education of appropriate patients to direct and enhance access to ASC services and improve patient, family and physician satisfaction.

    c. Develops, interprets and recommends implementation of policies, procedures, regulations, programs and recommended practices to ensure requirements are met.

    10. Duties and Responsibilities

    1. Responsible for the overall administration and operations of the Total Joint Program under the direction of the Director of Nursing
    2. Ensures the timeline and key deliverables of the implementation plan are achieved
    3. Works in collaboration with physicians and physician office personnel for marketing and outreach to conduct community education to increase awareness and grow market share.
    4. Conducts pre-surgery education classes for patients and caregivers
    5. Conducts face-to-face evaluation of patient and ensures patient meets admission criteria for ASC
    6. Works closely with Anesthesia to ensure face-to-face evaluation takes place.
    7. Coordinates all aspects of home care related to Home Health, DME and PT and ensures all meet DOS criteria
    8. Works with Managers to ensure patient satisfaction and quality of care throughout patent experience
    9. Accountable to maintain education materials to educate patient, caregivers, physicians, office staff and ASC staff to ensure optimal care of patient
    10. Works closely with surgeons to ensure protocols are followed and educational materials are current
    11. Works in conjunction with BHC Total Joint Program to ensure standardization as much as possible
    12. Works with Clinical Managers to develop, define and evaluate competencies to ensure staff is knowledgeable and able to care for total joint patients throughout the patient experience.
    13. Maintains a flexible schedule as necessary to meet patient needs
    14. Participates in development, implementation and evaluation of Total Joint Program mission and goals
    15. Serves as liaison and subject matter resource to physicians, staff, patients, families and professional organizations to ensure optimal patient care is provided
    16. Works closely with QA Coordinator to develop data collection methods and processes for tracking and trending quality outcomes
    17. Works as a mentor for professional practice and clinical team development related to total joint procedures
    18. Demonstrates age appropriate care across lifespan with emphasis on adult needs
    19. Participates in appropriate clinical meetings and committees to ensure continuum of care
    20. Communicates professionally with patients, caregivers, surgeons, physicians, vendors and staff
    21. Adheres to the Credo and Policies and Procedures of the ASC.
    22. Performs other duties as assigned
    23. Check and respond to emails in a timely and efficient manner
    24. Works closely with Administrator to promote the facility

    Qualifications

      1. Bachelor's degree in related field required; Master's Degree preferred
      2. Graduate of accredited school of nursing
      3. Current Florida Registered Nurse License
      4. Current BLS, ACLS
      5. Advanced Certification preferred
      6. Ability to generate reports and patient materials related to total joint program

    Requirements

      1. Strong knowledge of computers and MS Office programs
      2. Minimum of 5 years in supervisory/management position
      3. Previous experience in start-up of program (Dialysis, Home Health, etc.) a plus
      4. Ability to generate reports and patient materials related to total joint program
      5. Strong ethical and moral character references
      6. Excellent Leadership skills
