    Your Role:

    • Collaborate with our healthcare team to develop and implement personalized home care plans.
    • Monitor patient progress with diligence and empathy, ensuring precise and thorough documentation.
    • Administer medications and provide essential wound care, demonstrating your expertise and compassion.
    • Utilize advanced technology like HomeCare HomeBase to streamline documentation and improve communication.

    What You Bring:

    • Active RN licensure in Kentucky.
    • At least one year of recent experience in home health.
    • Proficiency in OASIS documentation and HomeCare HomeBase, or a willingness to learn.
    • Experience with a home-to-home workload is a plus.
    • Current BLS Certification from the American Heart Association.
