- Deliver quality hospice and palliative care services to patients in their homes.
- Performs initial assessment of patient's needs to determine eligibility into hospice services.
- Serves as a clinical resource for referral sources to patients/family members.
- Facilitates end of life conversations and decision-making with the family or caregiver.
- Represents our organization well when communicating with patients, families, and physicians.
- Must be able to work a four 10-hour days schedule: Mon/Tu/Th/Fr.
- Must have an active RN license to practice in the State of Maryland.
- CHPN certification preferred.
- A minimum of 3 years of acute care and/or hospice experience.
- Ability to demonstrate knowledge and skills necessary to provide meaningful care to communicate with the terminally ill.
- Ideal candidates will possess excellent communication and customer service skills.
- Great attention to detail and strong organization skills.
- Proficient with electronic medical records and Microsoft Office 365.
- A valid driver's license and reliable, independent transportation to provide care to hospice patients in Montgomery County.
