    PHP Nurse Practitioners have a passion for caring for seniors and other residents in a variety of healthcare settings that can include, but are not limited to, skilled nursing facilities, assisted living, and independent living communities.

    They deliver quality care in a high touch, person-centered care model and collaborate with the interdisciplinary team to ensure that all residents receive the right care, at the right time.


    Job Summary:
    We are seeking a dedicated Nurse Practitioner (NP) to join our dynamic team on a full-time basis.

    The ideal candidate will have a strong background in multispecialty care, with expertise in nephrology, cardiology, endocrinology, pulmonary, and infectious disease.

    Training opportunities are also available for less experienced APPs. This role offers the opportunity to work in a collaborative environment and make a meaningful impact on patient outcomes.


    PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS
    Duties of all PHP Complex Nurse Practitioners


    • Provide direct patient care, including assessment, diagnosis, and treatment, under the supervision of the attending physician.
    • Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients with complex medical needs.
    • Conduct regular rounds and follow-up visits to monitor patient progress and adjust treatment plans as necessary.
    • Educate patients and their families about their medical conditions, treatment options, and self-care techniques.
    • Participate in clinical meetings, case conferences, and quality improvement initiatives to ensure the delivery of high quality, evidence-based care.

    EDUCATION and QUALIFICATION:

    Required:

    • Master's degree from an accredited Nurse Practitioner program.
    • Current NP license in the state of Maryland.
    • Minimum of two years of clinical experience is preferred, but not mandatory.
    • Strong clinical assessment and diagnostic skills.
    • Excellent communication, collaboration, and interpersonal abilities.
    • Commitment to providing compassionate, patient-centered care.
