    Miami Valley Hospitalist Group FT/ VARIESGeneral Summary/Responsibilities:The Hospitalist Patient Care Assistant (PCA) is responsible for assisting in the administrative duties required to support the daily operations of the inpatient Premier Health Hospitalist program.

    This person is responsible for assisting with care coordination from admission to discharge.

    The PCA works under the supervision of the Practice Manager and Medical DirectorOur PCAs are an integral part of our office, ensuring safe patient handoffs and coordinated post-acute care for our patients.

    Essential Duties & FunctionsDuties performed to ensure safe patient handoffs include, but are not limited to:
    Assign inpatients to a physician rounder, ensuring lists are equal and unit-based rounding is maintained. This includes coordination with hospitalist nurses.
    Distribute and coordinate admissions, consults, direct admissions, and communications for the Hospitalist Group physicians and advanced care practitioners. This includes communication with the Access and Transfer Center, ED providers, and providers from external facilities.

    Identify PACU patients from all MVH campuses and distribute to appropriate MVHG practitionerMaintain accurate and up-to-date documentationDuties performed to assist in coordination of post-acute follow up care include, but are not limited to:Assist with completion of patient forms including FMLA, disability, oxygen orders, home care orders, pharmacy queriesAssist unattached patients in connecting with primary care physician and appropriate specialists for follow-up careCommunicate with physicians, advanced practice providers, hospitalist nurse liaisons and the hospital's care management team to address obstacles regarding patient's discharge planOther Duties & FunctionsOther duties as assigned The above duties and responsibilities may be essential job functions subject to reasonable accommodations.
