- Provides in-home evaluations, noting items that need additional support, providing resources, and a commitment to confidentiality.
- Plans and directs ongoing care in collaboration recognizing individual needs of clients.
- Provides information to members and providers regarding mental and physical health services and community resources.
- Recommends and arranges discharge plans, aftercare referrals and community resources when client is being discharged from hospital, SNF, and/or rehab facilities.
- Consults with providers to assist members with transportation, employment, and daily living needs as they relate to individualized care plans.
- Works closely with nurse care managers, discharge planners, and peers while maintaining tactful and patient composure.
- Attends client care conferences and develops interdisciplinary plan of care for each client.
- Attends daily, weekly, monthly meetings per agency policy and ensures follow-up on any tasks in a timely manner to ensure client care needs are met.
- Completing monthly reports and documentation for each client.
- Bachelor's degree in psychology, human development, or social work, BSN or higher
- Minimum of two (2) years combined direct behavioral health clinical and managed care experience.
- Desired - Have either a current, valid, unrestricted independent license in behavioral health or nursing and practice within the scope of their licensure in the state of Washington and/or Oregon.
- Demonstrates experience establishing relationships and effectively engaging with members and providers through telephonic communication to obtain necessary information and facilitate care in multiple settings.
- Demonstrates work experience to influence and negotiate to effectively manage patient care and health care outcomes.
- Demonstrates work experience meeting strict deadlines and established cycle times through effective prioritization and follow-up skills.
- Excellent communication skills, both verbal and written.
- Demonstrates ability to relate effectively to behavioral health and medical treatment providers, clients, and family member as well as other professionals involved in the treatment of the client.
- Familiarity with motivational interviewing is preferred.
- Must be able to adapt to a flexible work schedule.
- Ability to relate positively, effectively, and appropriately with clients, families, community members, and other agency staff.
- Must possess special interest in, and a positive attitude about, working with those with mental health diagnoses and the elderly.
- Demonstrates basic computer knowledge and ability with an aptitude to learn agency applications.

Description
Integrative Health Network is currently hiring for a contract position (starting at 1/4 FTE with the potential to increase to 3/4 FTE) seeking a dynamic Care Manager I to join our expanding team in the areas of Southwest Washington & the Portland Metro area.
The Care Manager I will be responsible for providing all-inclusive mental health and substance use services provided during in-home visits; assists with coordination of care between primary care providers, supportive staff, private care agencies, and additional team members.
This is a contracted 1099 position that requires you to have internet access, provide a work cell phone, and reliable transportation.
