    Job DescriptionResponsible for successfully on boarding new clientele and managing current clients' care needs

    SUPERVISORY RESPONSIBILITIES:
    Ensure home care aides understand client care needs.
    Provide one-on-one, on-the-job training for home care aides.


    ESSENTIAL JOB FUNCTIONS:
    Respond to prospective client phone, online, and in-person inquiries-provide information and conduct consistent follow-up.
    Conducts assessments with prospective clients-address questions, build relationships with families, and establish service with the Agency.
    Accurately complete and document new client assessments and service agreements.

    Deliver ongoing client support including regular client supervisory home visits, consistent communication with client families, re-assessment of client condition, and management of established care plan.

    Provide additional support to new clients by introducing caregivers at the first shift and following up after first week of service to ensure client satisfaction.

    Collaborate with Care Coordination to ensure client care needs are met and concerns are addressed promptly and thoroughly.


    OTHER JOB FUNCTIONS:
    Provide emergency after-hours, on-call support.
    Other duties as assigned.


    KNOWLEDGE, SKILLS AND EXPERIENCE:
    High school diploma required; bachelor's in business, Healthcare Administration, or Social Work preferred.

    Minimum two (2) years' experience in healthcare, elder care, or related field required; Current LVN, CNA, CSA, or other Senior Care designation a plus.

    Strong computer and internet skills including all MS Office suite (Word, Excel, Outlook, PowerPoint) required.
    Strong customer support orientation (internal and external customers) demonstrated professional demeanor, and the ability to maintain confidential information required.
    Excellent interpersonal and communication skills required.
    Ability to work both independently and in a highly collaborative environment.


    PHYSICAL DEMANDS:


    Ability to remain in a stationary position 60%; Move about the office 10%; Travel within territories 30%.Ability to relay accurate information to employees, clients, and coworkers; Receive detailed information through oral communication.

    Close vision, distance vision, and the ability to adjust focus are required.
    Frequent, repetitive use of hands and fingers.
    Occasionally assists with transfers of ambulatory clients.
    Occasionally bends, reaches, stoops and twists.
    Occasionally lifts and/or moves up to 25 pounds.


    WORK ENVIRONMENT:
    Frequently indoors in a temperature-controlled office, client home, or facility within territories.
    May occasionally work outside.
    The noise level is typically low to moderate.
