    The Dialysis Licensed Medical Social Worker position involves addressing the psychosocial needs of dialysis patients to enhance outcomes and improve their quality of life.

    Schedule: Full-time position from Monday to Friday, with shifts starting at 8:00 am; weekends off

    Compensation: Salary ranging from $59,000 to $78,000 annually, depending on dialysis experience

    Responsibilities

    Expectations:

    • Provide support for patients dealing with emotional, financial, and lifestyle changes due to chronic kidney disease
    • Help patients access community resources for various supports
    • Advocate for patients in obtaining healthcare and other benefits
    • Educate patients and families on treatment options
    • Assist in decision-making regarding care
    • Arrange translation services
    • Assess support systems and provide necessary referrals
    • Offer counseling services for coping with kidney failure
    • Create individualized care plans in collaboration with the care team
    • Adjust care plans as needed
    • Educate the team on emotional aspects of dialysis
    • Advocate for patients at various levels
    • Participate in quality improvement initiatives
