- Orients new patients to the PAWH program and with health center services.
- Links and supports patients to appropriate medical care.
- Conducts home visits as needed.
- Conducts reminder calls and follow-up for missed appointments.
- Provides support, education, and adherence assistance through individual or group activities.
- Assists patients with accessing benefits/entitlements.
- Assists patients in navigating and accessing referral sources as needed.
- Escorts patients to medical visits or other health and human service appointments.
- Participates in case conference, quality improvement projects, and collaboratives to share consumer perspectives.
- Participates in ongoing training supervision, and professional development opportunities on for skill development.
- High School Diploma/GED Required; Associate Degree preferred

The Patient Support Specialist assists patients in navigating the health care system, links patients to medical care, provides education and support to promote engagement and retention in care. Utilizes motivational interviewing offering patient centered care to promote medical adherence. Assists patients in reducing barriers to care and improve health outcomes. Shares lived experience strategies with patients to address issues of chronic diseases. Participates in monthly PAWH workshops and activities.
