- Peer certification will need to be obtained within the first six months of employment
- Use agency vehicle to transport clients to appointments, meeting and recreational activities as assigned.
- Assist clients in articulating personal goals for recovery and whole health by supporting client's in identifying strengths to add to client's healthy habits.
- Assist clients in working with their case worker and/or Psychiatrist.
- Assist clients in setting up and sustaining a schedule of attendance for self-help support groups, productive, positive activities.
- Provide client-centered services, focusing on the present. Listen, respond empathically and engage in problem solving.
- Serve as a recovery agent by providing and advocating for any effective recovery-based services that will aid the client in daily living.
- Assist in obtaining services that suit the individual's recovery needs by providing names of support in the client's community.
- Document the following on the client's treatment/recovery plan: identified person-centered strengths, needs, abilities, and recovery goals, progress made toward goals.
- Identify, assess and respond to crisis situations.
- Assist clients in attaining community stability and reaching their recovery and rehabilitation goals, including helping clients to find and keep employment.
- Increase natural supports - Engage, offer support, and mental health information to clients' family members.
- Provide community-based outreach and engagement in homes and other environments.
- Counsel clients individually to set, track and achieve rehabilitation and recovery goals.
- Listen, respond empathically and engage in problem solving. Identify symptoms and assist clients in self-management skills for mental illness and substance use.
- Maintain accurate and secure summary information based on documentation in computer files and databases and follow the HIPAA Privacy Rules guidelines for protecting client's medical records.
- Meet and record productivity and outcome standards of the program.
- Ability to travel and work at various program locations. (Marina, Monterey, Salinas)
- Collaborate and coordinate services with multidisciplinary staff team members, Psychiatrist, Nurses, Family Partner, Peer and other staff to provide effective services.

JOB DESCRIPTION
The Wellness Navigator for the Assertive Community Treatment Team (ACT) will be responsible to manage and lead the ACT team in providing crisis care, case management service delivery, individual rehabilitation activities, skills building, transportation and all aspects required to help clients succeed.
The incumbent, as peer providers, will draw upon their personal, lived experience with mental health challenges to assist others by connecting clients with resources within the mental health community.
The Wellness Navigator will serve as a role model to mental health clients; exhibit competency in personal recovery and use of adaptive coping skills; serve as an advocate; provide information and support to help clients access mental health and/or substance use services.

SALARY
$ $30.75 per hour. Non-Exempt; Full Time; 20 hours/week; Eligible for overtime.

QUALIFICATIONS
Required:
Bilingual in Spanish. High School Diploma or GEDApplicants MUST have personal experience of recovery from a mental health challenge. General knowledge of signs and symptoms of mental health challenges (i.e. auditory and visual hallucinations, aggressive talk and behavior, thoughts of self-harm towards others, isolation), ability to assist the client to address symptoms using strategies such as positive self-talk, and other techniques, flexibility, ability to work with a multi-disciplinary team of medical professions, good interpersonal skills, good English and Spanish communication, both verbal and written. Ability to maintain appropriate boundaries while working in close inter-personal relationships. Ability to learn and apply principles of social rehabilitation, mental health and substance use recovery.
Ability to be organized, responsible, flexible, empathic and able to relate effectively with clients, other staff, professionals and members of the community.
Drivers license for at least two years and driving record acceptable to insurance carrier.Valid California driver's license; auto in safe operating condition; auto liability insurance; good driving record; criminal record clearance; proof of authorization to work in the United States as required by Immigration and Reform Act of 1986.
Ability to work independently with minimal supervision.Preferred:
Associate or Bachelor's degree. Proficient in Outlook email correspondence and computer software's. Knowledge of community resources.
Ability to; work as a member of a team, accept supervision, maintain appropriate boundaries while working in close inter-personal relationships; learn and apply principles of social rehabilitation; organized, responsible; flexible; empathic and able to relate effectively with clients, other staff, professionals and members of the community.

walking, standing, sitting, stooping, climbing stairs.
Capable of the following intermittent activities: stooping, kneeling, lift up to 25 pounds or more and bending in the performance of infrequently performed office duties.
