    Description

    Position Overview:

    • Oversees Special Treatment Program (STP) referrals from County placement teams to verify all patient information for completed documentation files.
    • Works closely with County placement teams, discharge planners, and medical entities to assist with patient placement into STP programs.

    Essential Functions:

    • Facilitates communication and follow-up with County placement teams and discharge planners.
    • Oversees pre-admission paperwork for referral file completion.
    • Develops and implements policies and procedures for STP Admission Process.
    • Manages STP database/tracking system for bed availability.
    • Coordinates patient placement with STP Facilities and Conservators.
    • Builds and maintains relationships with County placement teams.
    • Supports Operations Director of Behavioral Health Services.
    • Maintains a list of patients referred to all STP locations.
    • Protects residents' privacy and confidentiality.
    • Reports any potential resident abuse.
    • Other duties as assigned.

    Knowledge, Skills, and Abilities:

    • Knowledge of medical, social, and financial information related to nursing care.
    • Effective communication with supervisors and County placement staff.
    • Proven success in providing timely customer service.
    • Skills in Microsoft Office 365 or similar platform.
    • Leadership skills for department operations.
    • Ability to manage multiple tasks efficiently.
    • Cultivate positive relationships with staff and peers.

    Education and Experience:

    • Bachelor's degree in business, marketing, or related field preferred.
    • 2 years of experience in nursing or hospital admissions.
