Job summary
The Community Health Worker – Home Visiting will be assigned full-time to conduct home visits with pregnant and new parents.
This position will follow an evidence-based curriculum designed to strengthen parent-child bonds and prevent child abuse.Under general supervision performs a variety of paraprofessional and office support duties related to the improvement of family functioning and child adult services enters data into a variety of software systems updates maintains specialized social records;
- Two years of paraprofessional and office support work in a Public Health Health Services Social Services or Behavioral Health program;
- An Associates degree with a major in health sciences social/behavioral sciences education nursing or closely related field;
