    Job Summary

    The eligible register developed from this posting may be used to fill future FT and PT vacancies within this classification.

    Performs highly responsible and professional level social work or nursing involving assessment and support planning, and eligibility determination for publicly funded long term services and support programs for seniors and persons with disabilities.

    Essential Duties

    Conduct comprehensive online MnChoices Assessments for clients by evaluating their physical, mental, social and environmental health status.

    Determine applicants' risk of institutional care and their level of care according to state and federal criteria.

    Analyze assessed data and assist applicants in identifying relevant goals along with specific supports that will enhance individuals' abilities to meet goals and live in the community for as long as possible.

    Maintain broad knowledge base related to payer sources and community-based services.

    Interprets programs to program participants and refers to appropriate community resources.

    Determines eligibility for home and community-based programs (AC, BI, CAC, CADI, DD, EW waivers, PCA) based on individualized program participant needs and wants.

    Coordinate the provision of long-term services and supports to ensure 1) appropriateness, 2) quality, 3) cost effectiveness.

    Maintains working knowledge of Federal and State laws and regulations and performs work accordingly.

    Works the days and hours necessary to perform all assigned responsibilities and tasks. Must be available (especially during regular business hours) to communicate with subordinates, supervisors, program participants, vendors, and any other person or organization with whom interaction is required to accomplish work and employer goals.

    Performs other related duties as required or assigned.
    Minimum Qualifications

    This position can be filled by either a Registered Nurse, Public Health Nurse, or Social Worker:

    For the Nurse, the qualifying credentials and experience is as follows:
    • BS in nursing w/ a certification in public health nursing; or
    • Current license as a Registered Nurse and at least 2 years of home and community-based experience
    For the Social Worker, the qualifying credentials and experience is as follows:
    • Requires the equivalency of a Bachelor's Degree with a major in any field, with at least two years of experience as a social worker in a public or private social services agency.
    Bilingual abilities are desirable. Must possess a valid driver's license and provide a reliable means of transportation for the performance of work responsibilities.
    Selection Process

    Selection for this position will be based on a minimum qualifications screening and rating of training and experience. Top candidates will be forwarded to the hiring manager or supervisor for consideration for interview and/or additional assessments. Final selection will include a background check and approval by the County Board.

    Note - Intermittent, Temporary, and PT positions less than 20 hrs per week are not eligible for benefits.
