the program With shelter food basic safety issues being met personal development individual needs can be assessed restoration process may begin Location: French Camp Benefits Health dental Cal Savers vacation sick leave About this Position The Case Manager delivers services with client-first focus within team setting This position supports client progress coordinates workforce development partners monitors compliance including probation-related obligations connects clients internal external resources Duties Client Intake Needs Assessment Conduct comprehensive assessments justice-involved individuals identify needs housing employment mental health substance use barriers particularly homelessness Develop Individual service plans measurable goals areas employment housing education behavioral legal obligations Maintain updated records case plans agency standards funder requirements Regular one-on-one meetings assess progress offer guidance revise goals strategies as needed Support clients navigating criminal justice system especially those on probation assist court probations check-ins classes appearances Re-entry Housing Stability Planning Develop re-entry stabilization tailored each client circumstances reducing recidivism Resource Navigation Referrals Connect community-based services emergency shelter transitional permanent supportive housing workforce behavioral public benefits Resources Systems Collaboration Coordinate services communication parole/probation correctional institutions outreach teams service providers continuity care compliance supervision terms Client Advocacy Advocate systems addressing stigma systemic barriers unhoused populations Crisis Management Stabilization Respond crises providing immediate supports Accurate confidential case notes data tracking grant funding guidelines Reports summaries required internal team meetings funding compliance monitoring Multidisciplinary team meetings trainings strategic planning trauma-informed re-entry PHYSICAL REQUIREMENTS Employee regularly sit keyboards handle feel talk listen others hear understand speech normal levels telephone Speak small large groups telephone Job Type Full-time Pay $51 $58 691 per year Benefits k Dental insurance Health insurance Life insurance Paid time off Vision insurance Ability Commute French Camp CA Required Location In person Physical demands representative successfully essential functions Reasonable accommodation enable disabilities
