Provides tools and information to assist the patient's family after discharge. Documents the education provided and the patient/family response to the education. -Systematically evaluates quality and effectiveness of Nursing Practice and uses the results of quality improvement activities to initiate changes in individual nursing practice and care delivery. Engages in self evaluation of practice, identifying strengths and areas in which professional development would be beneficial. Takes accountability to increase knowledge and enhance skill. Uses logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems. -Nursing practice reflects the importance of making a meaningful connection with patient/family. -Seeks to develop a helping, trusting, and caring relationship by being present in the moment of interaction, listening, communicating, engaging, and anticipating the patient/family needs.
