- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans

- Must have a valid RN/LPN license to practice in Washington State
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills

The right candidate will have experience in the administration and documentation of home-based care, including patient assessment, planning, implementation, observation and intervention.
