Human Touch Home Health is seeking an experienced Occupational Therapist (OT) for a PRN (as-needed) home health position serving Allentown, Bethlehem, Easton, Carbon County, and the greater Lehigh Valley region.

What You'll Do
Perform comprehensive in-home OT evaluations and reassessments
Develop and implement individualized treatment plans
Provide skilled interventions to improve functional independence
Educate patients and caregivers on adaptive techniques and home safety
Complete accurate and timely documentation (OASIS as applicable)
Collaborate with physicians and interdisciplinary care team members
What We're Looking For
Active Pennsylvania Occupational Therapist license
Minimum 1–2 years OT experience (home health preferred)
OASIS experience preferred
EMR experience (WellSky/Kinnser or Homecare Homebase a plus)
Strong documentation and communication skills
Valid driver's license & reliable transportation
