In this role, the Occupational Therapist in Home Health will be responsible for evaluating and assessing patient care requirements concerning functional abilities, daily activities, fine motor skills, home environment evaluations, adaptive equipment needs, and other occupational therapy services based on specific medical conditions.

Deliver services within the legal scope of Occupational Therapy practice, following state regulations, the prescribed care plan, evidence-based methodologies, and in collaboration with other healthcare professionals.
Assess the patient's functional status, overall health condition for CMS documentation, and occupational therapy necessities. Collaborate with physicians to develop the therapy care plan.
Monitor and document the patient's response to treatment, as well as any changes in their condition, reporting observations to the nurse supervisor and physician.
Conduct comprehensive patient assessments by utilizing diagnostic tools to evaluate functional capabilities and predict prognosis.

License Requirements:

Active Occupational Therapy license in the state
CPR certification
Valid driver's license, car insurance, access to a reliable vehicle, or use of public transportation
