(Hospice and Palliative Medicine)

About this role.

As an Advanced Practice Provider in Hospice Care, you will work with two other physicians to provide top-notch care and patient experience.

Throughout the day, you will collaborate with Social Workers, Case Managers, and Chaplains.

• You would serve the community of St. George and Hurricane, Utah
• Your hours will be flexible with no call
• You would see a maximum of three patients a day in their home
• You could expect to work 10-16 hours/month

What you'll bring.

• Master's degree or higher in Nurse Practitioner or Physician Assistant Studies.
• Current state licensure as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA).
• Ability to successfully complete Intermountain Health's credentialing process
• Current driver's license, insurance, reliable transportation, and an acceptable driving record to travel to deliver care where needed in a safe and timely fashion.
