    E ssential Functions and Objectives:


    S upports and strengthens the link between community-based organizations, businesses, faith communities and philanthropic groups to schools, communities and families.

    Supports the systems-driven approach to case management for the multi-generation services offered to participants out of the Community Hub.

    Responsibilities include creating a welcoming and safe environment for all Community Hub participants by building positive relationships; partnering with participants to identify and work towards their long-term goals for family stability; connecting community members with wraparound services to support them in building skills and accessing resources; and utilizing community center practices, procedures and data systems to deliver high-quality customer service and demonstrate program outcomes.


    • Serves as a Community Hub team member, providing program support for multigeneration services for adults with engagement in longterm sustainable employment and other individualized services.
    • Serves as a case manager of Community Hub students, following systems and structures to ensure participants have access to postsecondary education or longterm employment; supports participant caseload for case management of followup services to ensure Community Hub student and family success in achieving selfsufficiency goals; performs daily data collection and program implementation for Community Hub participants.
    • Completes Community Hub intake, followup, and satisfaction survey processes with participants.
    • Encourages and accommodates a variety of learning and participation styles, building on staff and community members' strengths; draws upon the experiences of participants or collaborators to include diverse perspectives in any given intervention and positively collaborates with the Community Hub and school staff members to ensure highquality delivery of services.
    • Works collaboratively with department staff to implement researchbased systems to serve families with highquality customer service and individualized support for family and student success.
    • Supports systems for quality control aligned with grant compliance for Community Hub data collection and operations; supports daily operations and program support of the Community Hub, including daily phone calls and responding to voicemails; assists walkin participants and scheduling appointments and leads basic needs services for Community Hub students and participants.
    • Leads special events or new project initiatives to maximize center capacity and to further engage participants in services; supports the ongoing expansion of services offered at the Community Hubs and leads outreach within regional communities and collaborates with other department staff to increase awareness of services offered at the Community Hubs.
    • Works collaboratively with other Community Hub staff and between community centers supporting the implementation of researchedbased systems across the Community Hubs to ensure highquality programming throughout Denver.
    • Meets regularly with the Community Hub team to engage in professional development and to support best practices for the engagement of participants; engages in skills training across the department to
    ensure quality control and efficiency when working with grant-driven programs.

    K nowledge, Experience &

    Other Qualifications:


    • Three (3) plus years of experience in community work or related.
    • Proficiency with Google Workspace.
    • Effective time management and organizational skills.
    • Effective verbal and written communication skills and excellent presentation skills.
    • Selfstarter/leader who can anticipate department needs in a proactive (versus, reactive) manner.
    • Effectively handles multiple demands and competing deadlines, even with interruption.
    • Ability to set goals, work independently and drive results.
    • Ability to work in a multiethnic and multicultural environment with district and school leaders, faculty, staff, and students.
    • Ability to work under pressure and adapt to changing circumstances with a positive attitude.
    • Ability to be flexible with work hours in order to meet the schedulerelated needs of parents, including early mornings, evenings, and weekends.
    • Bilingual (English/Spanish) proficiency desired.
    E ducation Requirements:

    • High school diploma or equivalent.
    • Bachelor's or relevant work experience (preferred).
