    Job Description

    Community Action Committee of Cape Cod & Islands is seeking a Re-Housing Case Manager to work directly with families staying at its shelter for women and children. The incumbent will work directly with families, providing support services to find permanent housing for stabilization. Must be available nights, weekends and holidays.

    Case Management Responsibilities:

    The Case manager will develop Re-Housing Plans. This plan incorporates, budgeting for future housing expenses, job search, housing search, education resource support, addressing any barriers to housing monthly. Additionally, Case Manager is a liaison for support services and referrals while working with families to advocate and coordinate services.

    Qualifications:

    · Associate Degree (Preferred)

    · At least 2 years experience in case management or in a related field developing budgets, providing support resources, addressing barriers to housing, and tracking educational, housing and employment logs.

    Strong verbal and written communication skills in English.

    · Bilingual Spanish (Preferred)

    · Computer Proficiency

    · Client service minded, able to uphold confidentiality.

    · Able to communicate effectively with people of various backgrounds.

    . Attention to detail and ability to multi-task.

    · Able to prioritize and meet deadlines.

    · Excellent organization skills

    · Reliable transportation

    Salary: $39,000- $45,000 annually plus benefits

    Shift and Schedule: Monday – Friday 12:00 p.m.-8:00 p.m. Full time 35 hours
