Job summary
The Care Coordinator is responsible for delivering counseling, case management, discharge planning and related duties to assigned patients.
This includes assessment, treatment planning, individual and group psychotherapy,
safety advocacy community referrals crisis intervention management of discharge
planning and treatment team meetings.
