Job DescriptionJFSWNY is growing We are accepting applications for a full-time Adult Health Home Care Coordinator at our Niagara Falls location.

WHAT YOU'LL DO
In working with your clients, you'll be promoting client rights and access to social services, social security, mental health, and other support services. As part of the Care Coordination team, you'll manage all aspects of care for Health Home consumers. These responsibilities include tracking and arranging appointments, working in partnership with treatment providers, offering consumer advocacy and education, and coordinating other aspects of the consumer's community services.
Equally important are keeping accurate documentation, maintaining case notes, and completing (and monitoring) all required forms. For this reason, you must not only have proficient communication skills but must also have excellent writing skills and attention to detail.

The starting wage for the position is $21 per hour dependent on experience and language skills. This position offers great benefits including 4+ weeks accrued Paid Time Off (PTO), plus 13 paid holidays, excellent health insurance options (IHA), 401k, and much more. We also have a reduced 35 hour work week with an early close on Fridays
Since this position requires work in the community, you must have a reliable vehicle, a clean NYS driver's license and required liability insurance levels of 100/300K.
