    Title: Service Coordinator (Behavioral Health Case Coordinator)

    Job Duties: The Service Coordinator (Behavioral Health Case Coordinator) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Behavioral Health Case Coordinator) also coordinates the interdisciplinary team meetings and discharge planning efforts between the multidisciplinary treatment team. In this capacity, the Service Coordinator (Behavioral Health Case Coordinator) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

    Schedule:

    1. PM Shift: Wednesday-Sunday or Tuesday-Saturday

    Qualifications:

    • Prefer a bachelor's degree in psychology, social work, or a field related to mental health.
    • Two (2) years of full-time experience or part-time equivalent in a mental health setting.
    • Demonstrated knowledge of DSM-V and psychosocial rehabilitation and recovery principles.
    • Experience in working with health care and mental health regulatory agencies.

    About the Campus: Crestwood Center is a Mental Health Rehabilitation Center located in Angwin, amidst the beautiful hills of the Napa Valley. The facility provides a versatile array of classes for clients to participate in during their recovery program and encourages active participation in treatment planning from the moment of admission.

    The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity. Pay Range:$23—$25 USD
