    Provides complex care coordination and advanced care to patients with complex, acute, critical and/or chronic health conditions, typically with multiple disease states, in collaboration with or under the clinical supervision of a physician. Provides diagnostic, therapeutic, and preventive health care services for patients. Responsible for the patient evaluation, interpretation, diagnosis, and prescribing of appropriate medical and therapeutic measures. This job is based in an outpatient clinic, hospital-based outpatient department and inpatient hospital setting.

    Evaluates, diagnoses, treats, and manages acute, critical and chronic patients within an outpatient clinic, hospital-based outpatient department, and inpatient hospital setting.

    Obtains medical histories, conducts physical examinations and assesses and determines differential diagnosis.

    Interprets and evaluates patient data for the purpose of determining management and treatment of patients. Consults with physician as appropriate. Adjusts patient care plan by reviewing and monitoring treatments and therapy plans.

    Coordinates specialty care program for specific patient population in one or more of the following areas: mental health, and/or substance abuse/dependence, and eating disorders. Maintains patient schedule; assesses, diagnoses, treats, orders and prescribes based on patient needs.

    Assists with complex inpatient procedures in area of clinical specialty.

    Prescribes, recommends and regulates pharmacologic and non-pharmacologic therapies across multiple care settings, including controlled medications that are warranted by patient's diagnosis or changing medical condition.

    Provides complex care coordination, which includes managing a variety of ancillary services, laboratory findings, discharge planning, provider referrals and medical devices as needed.

    Orders, performs, and interprets procedures and diagnostic tests or studies, in the field of clinical specialty trained and determined competent by a physician, to manage simple conditions such as those produced by infection or trauma, as well as to assist in the management of more complex illness and injury. Examples include: injections, immunizations, suturing, wound care, screening for medical problems with point of care testing.

    Counsels, instructs and educates patients on physical and mental health including: diet, preventive health measures, health behaviors, self-care skills, disease, treatment options, and normal growth and development.

    Prepares, dictates and signs patient care plans and summaries of patient care. Maintains records according to established standards and guidelines.

    Licenses & Certifications

    Registered Nurse license issued by the state in which the team member practices, and

    Advanced Practice Nurse Prescriber license issued by the state of Wisconsin, and

    Nurse Practitioner (AANP) certification issued by the American Academy of Nurse Practitioners (AANP), or

    Nurse Practitioner (ANCC) certification issued by the American Nurses Credentialing Center (ANCC), and

    Adult Psychiatric & Mental Health Nurse Practitioner (PMHNP-BC) certification issued by the American Nurses Credentialing Center (ANCC), and

    Drug Enforcement Administration (DEA) registration issued by the Drug Enforcement Administration (DEA) Registration, and

    Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.

    Degrees

    Master's Degree in Nursing.

    Required Functional Experience

    Typically requires 2 years of experience in providing and coordinating care for patients with acute, critical and chronic health conditions.

    Maintains practice within both an outpatient clinic and inpatient hospital setting performing complex care coordination and advanced care to patients with complex, acute, critical and chronic health conditions.Demonstrated high level of clinical proficiency, and excellent decision-making skills.Demonstrated ability to work independently and as an effective member of a health care team.Excellent oral and written communication skills.
