    Full-time Palliative Care Physician / MUST LIVE IN LOUISIANA (New Orleans)
    Monogram Health is seeking a board-certified Palliative Care physician in LOUISIANA (New Orleans). The physician will be expected to manage patients who are on our conservative pathway. Treatment and oversight will occur through telehealth and locally in person.

    The palliative care physician will connect with patients who are appropriate for conservative care and engage with patients who have multiple complex co-morbidities.

    Monogram physicians are quality-driven. They apply their clinical expertise in a managed care, population health context. They serve patients across the country through a national telehealth platform. They promote palliative care where clinically appropriate, patient education, autonomy, and quality of life issues.

    The palliative care physician should be experienced in treating patients to reduce or control pain and in driving discussions about goals of care, prognosis, treatment, and advance care planning.

    This job is ideal for physicians who are forward-looking and embrace telemedicine and value-based care rewarding outcomes rather than utilization.

    The physician will work remotely from home and see patients in person locally when necessary. The physician will be licensed in multiple states after joining the company.

    Responsibilities:
    Collaborate with POD Medical Director to proactively identify CKD and ESRD members in predefined geographic regions to build patient panel
    Own outcomes of patient panel, driving both predetermined clinical and financial KPIs through interventions outlined below:

    Drive Clinical Interventions for Patient Panel:
    Treat panel of patients both in-home and virtually as necessary to drive healthcare outcomes
    Apply evidence-based criteria in determining the appropriateness and necessity of care; in particular, in determining whether dialytic or non-dialytic care is the correct evidence-based pathway
    Reduce pain in patients who have selected a palliative care pathway
    Round on patient panel (if admitted) in select hospitals within a defined geographic market
    Reduce preventable episodes of care including hospital readmissions
    Refer members to appropriate specialists to drive clinical care
    Develop key relationships with local providers (PCP, Nephrologists, Specialists) to improve market alignment.
    Work with local Monogram PODs to drive KPIs
    Participate in monthly sessions with Regional Medical Director and CMO concerning KPI expectations
    Backup coverage is to be provided by another Monogram MD team member MD must maintain active privileges at local dialysis cnters and hospitals
    Review Annual Wellness Visit Assessments
    Assist in the training of NPs and other providers

    Requirements:
    1+ years treating patients with CKD and ESRD
    Desire to practice in a renal care environment focused on population health management
    Experience applying evidence-based clinical criteria
    Experience in geriatrics or palliative care
    Skill in advanced care planning
    Experience with high-need Medicare Advantage and managed Medicaid populations
    Expertise in managing medication panels for complex patients
    Current state medical licensed
    Willingness to become licensed in multiple states
    Doctorate degree in medicine
    Board-certified in Geriatric or Palliative Care medicine
