Description

• The Homecare Assistant supports the patient in personal care, rehabilitation activities, nutrition, light housekeeping, and other therapeutic tasks as directed in the care plan created by a registered nurse or therapist.

• Follows the care plan established by the registered nurse or therapist.
  
  • Assists the patient with personal care.
  • Aids with medications as prescribed by physicians.
  • Conducts rehabilitation activities like ROM exercises, ambulation, and transfers.
  • Provides meals tailored to the patient's nutritional requirements.
  • Offers light housekeeping assistance.
• Ensures top-notch patient care and coordination.
  
  • Monitors and communicates any changes in the patient's condition or home environment.
  • Promotes appropriate levels of independence while ensuring the patient's comfort.
  • Communicates effectively if unable to fulfill planned tasks.
• Abides by agency policies and procedures.
  
  • Delivers safe, competent, age-appropriate care.
  • Completes documentation within specified timeframes.
  • Maintains productivity standards by performing duties efficiently.
  • Attends a minimum of 12 hours of relevant in-services annually.
• Keeps commitments, works autonomously, takes responsibility, adapts to change, sets high personal standards, remains focused under pressure, and meets attendance/punctuality standards.
• Embraces change, welcomes new ideas, assumes new responsibilities, handles pressure, adjusts plans to address evolving needs.

• Education
  
  • High school diploma or equivalent needed.
• Work Experience
  
  • Minimum of one (1) year experience as a healthcare aide in a hospital, extended care center, or home healthcare. Demonstrates emotional and mental maturity essential for cultivating positive relationships with patients, families, caregivers, supervisors, and agency colleagues.
• Certification/License
  
  • Required: Nurse Assistant certification (CNA) and CPR certification. A valid driver's license and proof of current vehicle insurance. Must obtain a Class E license within two weeks if assigned a company vehicle. Responsible for acquiring a Class E driver's license within two weeks if provided agency vehicle. Mandatory completion of Homecare Assistant competency evaluation post-hiring. BLS certification through American Heart Association mandatory.

• Ability to stand and walk for extended periods and navigate stairs. Proficient in receiving and conveying detailed information verbally, strong visual acuity, and adept at understanding written directions. Excellent interpersonal and organizational skills. Capable of lifting, transferring, and moving patients up to 35 pounds with varying degrees of assistance. Skilled in safe operation of vehicles.

• Frequent exposure to communicable diseases via blood and body fluids. Dealing with infectious diseases and challenging individuals occasionally. Handling sharps and other biohazardous materials. Exposure to varying environmental conditions. Working in diverse community settings.
