As Palliative Care Nurse Practitioners, we have the privilege of collaborating with a team of interdisciplinary providers to assist patients and families facing terminal illness. Nurse Practitioner Position Qualifications:

2 years of Nurse Practitioner experience in an acute care environment.
Masters of Science in Nursing - Family Nurse Practitioner.
Active unencumbered state NP license.
Current driver's license and auto liability coverage.
Detailed assessment skills as part of this role require recertification for Hospice patients.
Interest in a community-based role that requires travel between patient visits.
Drug Enforcement Agency (DEA) number Required.
National Provider Identifier (NPI) Required.

Nurse Practitioner Benefits & Schedule:

Monday to Friday 8:30 AM to 5:00 PM
