- Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate level of care
- Identify and initiate referrals for social service programs
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate with all stakeholders required health-related information to ensure quality coordinated care services provided expeditiously to all members,
- Care plans may vary but will consist on mandatory visits every 30 days unless otherwise indicated by physician orders;
